Creating a Patient Record

Patients -> Demographics

 

While there are a number of ways to enter a new client, the fastest is to click the New Patient button on the My Patients Widget:

 

The system will display a blank Demographics form you can begin to complete. Fields labeled with bold red text require that you enter information. You will not be able to save a record until you complete all required fields on all tabs. When you save the record, ClinicTracker will perform duplicate checking to make sure you have not entered duplicate patient records. 

 

The majority of fields on the Demographics form are self-explanatory. However, some of them involve nuances we've detailed below: 

 

Copying an Existing Patient

The Demographics form includes a useful feature if you're adding a spouse or other family member with similar information such as last name, address, phone, etc. To use this feature, go to: File -> Load From Existing Patient. You can then use the Find form to select the patient you are copying. After selecting the patient, the demographics form will be populated with pertinent information from the original patient's record. You can then make changes as necessary.
