Patient -> Patient Contacts

 

The Patient Contacts Module is one of the most frequently accessed forms ClinicTracker.  It is used to record Appointments, Collateral Activities, Medications, Progress Notes, and more.

 

Form Layout

The Patient Contacts module fuses two separate sections: Appointments and Collaterals.

 

Appointments are face to face visits, while Collaterals are all other contacts with a patient (phone calls, meetings, emails, etc.).

 

There is a separate tab for each of these sections. Unlike all the other forms, each tab represents a different function, so you cannot move freely between the tabs without first saving or clearing the record on the current tab.

 

On the bottom of the form is the Treatment History Box.

 

This section keeps a record of all previously entered Patient Contacts (both Appointments and Collaterals).

 

To load an existing record, double-click its entry in the Treatment History Box, and it will be filled in the appropriate section on the top of the form.

 

Once a record is loaded, assuming it has not been locked (see Locking A Record), you can make and save changes or delete the record and any attached items (Medications or Progress Notes).
