Patient -> Patient Contacts

Both tabs have a button labeled Medication. As noted above, an icon Medication next to the button indicates a Medication Record exists for the current contact.

 

To view or enter medication, press this button. Note that you will not be able to click this button until you have entered all required fields, because the main Patient Contact Record needs to exist before you can attach items such as Medications or Progress Notes.

 

Note: If you're licensed for ePrescribing, the Medication button will open the ePrescribing module. Otherwise you will use ClinicTracker's built-in Medication form described below.

 

Allergies

The top section of the Medication Form is labeled Allergies. This information is separate from the actual Medication Record, and will persist throughout the patient’s record. In other words, there is one Allergy section that appears on all of the patient’s Medication Records. To record allergy information, type in the Allergy box and click the Save button to the right.

 

Medication Fields

 

A Medication Record contains the following fields:

3. Total Daily Dose (mg) Total daily dose of medication.
4. Amount Prescribed Amount prescribed.
5. Refills Ordered Refills ordered.

7. See Progress Note for Details Check this box to indicate there are details listed on the Progress Note.
8. Change Medications as Follows Enter a description in the box or click to See Progress Note for Details.
9. Rationale for Change Enter a rationale for a possible medication change in the box or check off the appropriate boxes.
10. Discussed Risk-Benefits of treatment (or non-treatment) with patient/parent Check this box to indicate you discussed the risk-benefits. 
11. Discussed Instructions re: side effects and when to call Check this box to indicate you discussed side effects.
12. Medication Sample Check this box if you are giving a medication sample.
13. Finalize Clicking on the finalize button will allow the clinician to electronically sign the form. Explained in detail below.

 

 

Creating a Medication Record

Create a Medication Record the same way you would create any other record, that is by entering the information in the fields and clicking the Save button.

 

The Medication Module differs slightly in that clicking the Save button will actually queue a record for saving (as opposed to saving it right away). The record will show in the list on the bottom of the form, but will have an asterisks (*) in the first column to indicate that the record is queued for saving, but not actually saved yet.

 

Once you queue a record, you can enter another medication or make a modification to the queued record (by double-clicking its entry in the bottom list, making changes, and saving). You can queue up as many records as you want.

 

You may also create a new medication entry by loading it from an existing entry. Click on Actions -> View Recent Medication (or hit Control + R) to display a list of medications prescribed for the current patient in the last 30 days.

 

You may adjust the number of days to search back by entering a different number in the textbox just above the list. 

 

You can easily add a similar entry by double-clicking an item in the recent medication list to load the old information. You may then make modifications and queue the record for saving. This list can be shown or hidden at any time by choosing the View Recent Medications option in the Actions Menu.

 

Finalizing Medication Records

The reason for the somewhat complex queuing process is that you may be entering multiple medications. You don’t want to enter, sign, save, and print information separately for each individual medication.

 

It also gives you an extra opportunity to review what you have entered before signing and locking it.

 

Once you have queued all the medications, click the Finalize button that can be found just above the list on the bottom of the form. A form with all the queued records will appear.

 

Take this opportunity to verify the information you entered. If you need to make changes, close this form or hit the cancel button, double-click on the queued record, make the changes, click Save to put the record back on the queue, and start again.

 

If everything is correct, select your name from the Prescribing Clinician dropdown box and click Sign.

 

Once you enter your password and click OK, all the queued records will be signed and saved to the patient’s record and you will be returned to the Medication Form. The medications you entered now appear in the list with no asterisk in the first column. You can double-click one of the entries to view all the information and see your signature attached to the record.

 

Note:

 

Only staff members that have a check in the Can Prescribe box in their Staff Info Record will show as options in the Prescribing Clinician dropdown box.

 

Printing Medication Scripts

Once you enter a medication record, you can create a medication script based on that information.

 

If you have loaded a record (by double-clicking an item in the list on the bottom), you can go to File -> Print… or use the Print Icon PRINT on the top toolbar.

 

If you have items in the list on the bottom, you can use the Print Scripts Icon PRINT to the left of the list and choose to print all the items in the list, or only the selected item.

 

Regardless of which method you use, you have the option to print the script or export the information to Word.

 

The format of the output will depend on what was chosen in the Medication Scripts section of the Options Form, Formats Tab.

 

Due to New York State regulations, if you have set the Clinic State to be “NY” in the Options Form, General Tab, all numbers that are printed will be followed by the spelled representation of the number. 

 

For example, if you put a dosage of 50 mg, it will display on the output as 50 (fifty) mg.

 

Medication Listing

You can view all of the medications prescribed to a patient by date by going to Actions -> Medication Listing.

 

For a detailed view, click the View Complete Report button. This same form is available from the Patient Contacts Action Menu.
