Find Form

You will encounter the Find Dialogue often because it's the way ClinicTracker knows which patient record you want to select. It will frequently appear automatically when you open one of the forms. You can also access it by going to File -> Find, clicking Control + F, or clicking the Find Icon FIND

 

You have various options for locating a patient with the Find Form: 

  • In the top text box, start typing the patient's last name.  As you type, the form with display the first name that matches. 
  • Use the Rolodex-style letter tabs to display all the patients whose last name starts with the chosen letter
  • Display all names by clicking the tab with the asterisk (*)

 

Once you highlight the patient you're looking for, double-click on the name, hit the enter key, or press the Accept button to select the patient.

 

Note: By default, only active names display. To show the names of all patients in the system, click the Show Discharged checkbox on the bottom right corner of the form. Discharged names display with an asterisk at the end. The Show Discharged button will not show if you're on the QL (Quick-List) tab. 

 

Quick-List

One of the tabs on the Find form is labeled QL for Quick-List. It's a way for you to create a personal list of patient names you access often. To add a name to the Quick-List, simply select the name on any tab, right-click on it, and choose Add to Quick-List. It will now show on the QL Tab. To remove a name from the Quick-List, select it on the QL Tab, right-click and choose Remove from Quick-List. If you have any names in your Quick-List when you bring up the Find Dialogue, the QL Tab will be displayed by default. You can output a list of all the names on your Quick-List by right-clicking any name and choosing Print Quick-List. The Show Discharged button will not show if you're on the QL (Quick-List) tab. 

Advanced Search

If you cannot locate a patient using the standard Find Dialogue, you can use the Advanced Search Form by clicking Advanced Search on the top of the Find Dialogue. You will see a form that allows you to search based on the following criteria:

  • Status: Select Active, Inactive, or Both.
  • Date of Birth: Options for Is, Between, Before, and After.
  • Insurance Policy Number: Options for Is and Like.
  • First Name: Options for Is and Like.
  • Last Name: Options for Is and Like.
  • Hospital Number: Options for Is and Like.
  • Family ID: Options for Is and Like
  • Social Security Number: Options for Is and Like.

 

To add an additional search criterion, check the box next to the filter name to display an ON label to verify that this filter will be included. Then enter the search criteria in the corresponding textbox and choose the comparison operation. Selecting IS will search for an exact match. Selecting LIKE will search for a similar entry. For example, a Like comparison on “Sal” would match a patient named Salloum. Note that a search will combine all criteria together, so a patient will have to meet each criterion to be displayed as a matching result. To perform the search, click the Search button, and any matching names will be displayed in the list on the bottom. Select a patient by clicking the Accept button, double-clicking the name, or highlighting the name and hitting enter.

 

Unable to Locate a Name

If you are still unable to locate a name, you have several options:

  • If the person is Discharged, make sure you have checked the Show Discharged box.
  • If the person is still flagged as an Intake in their Intake/Patient Info Record, he or she will only show up in the Find Dialogue when accessed from the Intake/Patient Info Module. If you are trying to complete paperwork on an Intake, you will need to update their record to flag it as a Patient. That name will then show up throughout the rest of the program.
  • The person’s name could have been modified. Use the Advanced Search Form or contact other people involved in the case to ask about a possible name change.
