Acuity Questions

 

Several of the forms have a set of Case Acuity Questions and slider bars to use for the responses.

 

The questions ask "Compared to the average case, how much has this one been difficult because it involves..." with prompts for items like extra appointment demands, disruptions, etc.

 

Use the slider bars to indicate acuity levels.

 

Notice the last question is labeled as a Confidential Research Question. Clicking on the blue link will explain that the question is solely for research purposes, your response will not appear on any paper records, and you will not be held responsible for your answer. In addition, once you respond to this question and save the record, this section will be hidden the next time the record is displayed.

 

If you have set the option to Require Completion of Acuity Questions Before Printing on the Options form -> Preferences Tab, then you will need to enter a response for each question (even if that response is N/A) before previewing, printing, exporting to Word, or locking.

 

You can get graphical results from this data by running the Acuity Ratings Report using the Patient Contacts Actions Menu (for the current patient) or running the Rating Scale Data Report located at Admin Reports -> Clinic Productivity.
