Using Templates in Form Builder
Make form building easy, by starting with a pre-designed template.
About Form Builder Templates
There are templates for commonly-used forms available in Form Builder. This allows you to customize the form, by changing just a few things, rather than building a new form from scratch
Available Templates Include:
- Diagnostic Evaluation
- Patient Forms
- Treatment Plan
- Biopsychosocial
- Consent for Treatment
- Grievance Policy
- Group Contract
- Rights and Responsibilities
Editing a Template in Form Builder
Go to: Utilities > Form Builder > Form Builder
- Check the Show Disabled forms box in the lower right corner of the screen.
- Enter the name of the template in the Search box on the top right corner of the screen.
- Highlight the Template you'd like to use.
- Make changes to the template as needed.
- Click the Save Icon in the top right of the screen.
Publishing a Template in Form Builder
Go to: Utilities > Form Builder > Form Builder
- Ensure your form name is up to date and your form is enabled before distributing.
- Check the Show Disabled forms box in the lower right corner of the screen.
- Enter the name of the template in the Search box on the top right corner of the screen.
- Highlight the Template you'd like to use.
- Click the Edit Details icon on the top left of the screen (you may hoover over the icons to locate Edit Details)
- Change the name of the form if applicable
- Click Distribute Form
