Using Form Builder - Form Controls

When designing forms, you have many design options for each individual field.

About Form Controls

You can use Form Controls to organize how data is collected in your form. The purpose of this page is to introduce you to the various Field Controls that you have to choose from.

  • Reference will allow you to mirror a field from an existing Custom Form or Demographics. When using the new form, the Control Reference field will reflect the most recent data from the form and field that you referenced. Also, if the Access Method of the new form is Read/Write, you will be able to enter new data into the Reference Control field in the new form and the data will automatically be updated in the original form or demographics. 

    • Design a new form. Name your field "Phone Number." Use a Reference Control to reference the Phone Number Field on the Demographic form. Change the Access Method of the new form to Read/Write.
    • The first time you use your new form for a patient, the Phone Number field will be pre-populated with the phone number listed in demographics. When you speak with the patient, you learn there is a new phone number. When you enter the phone number in the new form and save the changes, the referenced field on the patient's Demographics will be automatically updated.

Standard Field Control Properties

The majority of field types have a number of options that you will need to configure. See below for a list of the most common field properties.

  • Field Type is set for you based on the field you've added or selected.
  • Display on Tab is used to select which of your created tabs you would like to add fields to. Toggle this option to move a field to a different tab.
  • Field Label is the name that you would like to give the field. The label will be displayed on the form you're creating unless you specifically Suppress Field Label (see below for additional information)
  • Description is used to add information about the field or provide directions. This will appear below the Field Label on the form.
  • Mouseover Text can be used to enter a tooltip when the mouse cursor hovers over the field.
  • Maximum Length can be used to set a limit to the number of allowable characters that a user can enter. (Leave blank for no limit)
  • List of saved fields will display the fields that you have configured and saved. The fields will be listed in the order that they appear on the form.
  • Form Display will display all of your fields in a rough example of how they will appear in the current form.
  • Output Formatting determines how your fields will be displayed when viewing, printing, and reporting.

Formatting Settings

  • Data Format sets the font for the field's contents (e.g. textbox entries, or dropdown list values)
  • Label Alignment determines where the Field Label is located on the field. Left, Center, or Right.
  • Suppress Field Label check this box if you would like the Field Label to be hidden on the form.
  • Don't Print check if you would like this field and its data not to appear when printing the form.
  • Don't Print if Empty becomes available if 'Don't Print' is checked. This will allow the field to appear when printed except when no data is entered, or no selection is made.
  • Report Field Name when using your form as a report source when building a custom report. This option is used to configure how this field will appear in the list of available fields.
  • Height (pixels) sets the vertical size of the field. Each field has its own default height. The most common setting is to leave the 'Auto' box checked by default.
  • Width Percentage determines the percentage that your field will occupy horizontally.
