Managing Form Builder Settings

There are a few important settings that can help you build more useful forms.

 

Combine Multiple Forms into a Packet

  • Go to Utilities > Form Builder > Patient Portal Form Packet.
  • Name Your packet.
  • Click on the Green + button to add an existing form to the packet.
  • Select the forms you want to add. 
  • Click Accept.
  • Save.

Add a Header or Footer for Forms

  • Go to Utilities > Form Builder > Configure Header and Footer.
  • Adjust settings as needed.
  • Preview.
  • Save.
