About Form Builder

Are you looking to customize your data capture capabilities beyond the stock notes and forms we've built into the system? With Form Builder the options for customization are endless. You can use this feature by itself to design forms, or pair it with other features like Report Builder, Compliance Builder, and Patient Portal to maximize your clinic's ability to adapt to its clinical and regulatory environment.

Adding Form Builder

To add Form Builder, or to request a demonstration, please use the blue Support Tab.

Next Steps: We recommend a training session to begin using Form Builder. For new clinics, this will be scheduled by your Implementation Specialist. If you add this feature after your implementation, you will be contacted by a Success Representative.

Give Form Builder Access to Staff

Go to Staff > Staff Info > System Permissions Tab > Form Builder
