Use this form to record Appointments, Collateral activities, Medications, and Progress Notes.


About Patient Contacts

The Patient Contacts module fuses two separate records tabs: Appointments and Collaterals. The Appointments are considered face-to-face visits, while Collaterals are all other contacts with a patient (phone calls, meetings, emails, etc.). 

Appointments Tab

The Appointments tab is where you will record Appointment Details, Diagnosis information, Progress Notes, Billing Details, Signatures, and more.

 

Appointment Details

To create an Appointment record from Patient Contacts, go to Patients > Patient Contacts.

  • Select the date and time of the Appointment.

  • The Billable Hours field will be filled once the Appointment record is completed. 

  • Select the Clinician, Supervisor, Program, and Place of Service.
  • Click the green plus sign to add the Service(s) and units. 

Diagnosis

  • Diagnosis Assignment
  • Edit, view, and add new diagnoses as needed
  • GAF(1-100)
  • Risk Assessment 
  • Medication Review

Billing Details

  • Status
  • Authorization 
  • Revision #
  • Session #

Progress Notes

Add Medication

  • Use this button to add patient prescriptions using the ePrescribing module.

External Contacts

  • Use this button to link your External Contacts. 

Travel Reimbursement

  • Use this button if your agency bills for travel expenses.

Treatment History

This section keeps a record of all previously entered Patient Contacts (both Appointments and Collaterals). To load an existing record, double-click its entry in the Treatment History Box, and it will be filled in the appropriate section on the top of the form. Once a record is loaded, assuming it has not been locked (see Locking A Record), you can make and save changes or delete the record and any attached items (Medications or Progress Notes).

Collateral Tab

  • To create a Collateral record, go to Patients > Patient Contacts > Collateral. 

  • Select the Date.
  • Select the Service
  • Select the Duration
  • Select the Program
  • Click the Signature button to sign.
  • Add Medication.
  • Send Notification. 
  • Save.
