Activating Clinical Scribe on Individual Forms
Use AI prompts in Form Builder to provide Clinical Scribe with specific instructions for completing Patient Forms and Progress Notes.
About Clinical Scribe
HIPAA-compliant and AI-powered, Clinical Scribe is integrated with your forms and allows you to dictate your notes or, even better, capture a live client session while you focus on your client (in-person or via telehealth). Clinical Scribe will automatically be aware of your clients’ charts, the form and field names in your form design, and the Mental Health/Substance Use treatment protocols in general. Form Builder AI Prompts will allow you to give even more detailed instructions about how you’d like your documentation to be completed.
Activating Clinical Scribe on a Patient Form or Progress Note
Clinical Scribe will be activated on a Patient Form or Progress Note if there is a form-level prompt and at least one field-level prompt set in Form Builder. There are three ways to set up your AI Prompts:
1. Automatically generate generic prompts with the push of a button.
2. Leverage ClinicTracker's AI-Prompt Engineering Service.
3. Write Detailed Prompts in your own forms.
Automatically Generating Generic Form-Level and Field-Level Prompts for All Fields.
Go to Utilities > Form Builder > select a form > Click Edit Details > Select the Generate AI Prompts button.
This will quickly enter generic prompts and will allow you to use and test Clinical Scribe with your forms. The auto-generated prompts will include the name of the field only and will not give added context or instructions. We recommend that you write detailed prompts that provide clear guidance to the Clinical Scribe regarding the context, format, and goal of the output. More detailed prompts will result in better content.

If you are short on time or experience, our Forms Expert team is available to set up your prompts for you. We will test prompts against your real documentation scenarios and ensure the outputs are accurate, formatted, and review-ready. We may ask for samples of your ideal notes, feedback from your team, and clarification around specific needs. Writing a Detailed Form-Level Prompt in Form Builder
Go to Utilities > Form Builder > select a form > Click Edit Details > enter your prompt in the General AI Prompt field > Save.
This prompt is a way to provide important context to Clinical Scribe about your form and your expectations. Below is a form-level prompt for a Psychological Evaluation. You will notice the prompt explains in natural language the context of the form as well as the content and format expected in the output.
Example Form Name: Psychological Evaluation.
Example Form-Level Prompt: Complete this Psychological Evaluation on behalf of a licensed mental health clinic serving adolescents in New York State. Use concise, objective, clinically appropriate language based on the encounter transcript. If a section is not addressed, return "Not Documented". Ensure that all clinical content is consistent with the Joint Commission elements of performance for psychiatric evaluations and aligned with New York State OMH documentation requirements
Writing a Detailed Field-Level Prompt
Go to Form Builder > select a Form > select a field > enter your prompt in the AI Prompt property for the field.
This prompt is a way to provide important context to Clinic Scribe about a specific field and your expectations. Below is a field-level prompt, instructing Clinical Scribe on how to complete a text field named Suicidal / Homicidal Ideation. You will notice the prompt explains in natural language, exactly the content and format expected in the output.
Example Field Name: Suicidal/Homicidal Ideation.
Example Prompt: Analyze the session transcript for any mention of suicidal or homicidal ideation. Indicate the type of ideation and the frequency. Outline any plan or attempt. Return a risk level of Low, Moderate, or High based on risk or protective factors discussed. Include the reason for the risk level identified. Complete in a narrative format and include any relevant patient quotes.

Identifying Clinical Scribe Forms
If Clinical Scribe is active on a form, this icon will be displayed in the Staff Portal next to the form in Paperwork, the Progress Note List in Appointments, and in the Form List in Form Builder.
