Setting Up Institutional UB-04 Claims

If your clinic submits Institutional UB04 claims, you can configure the settings according to your specifications.


Setting Up Institutional UB-04 Claim Transmission

If you send Institutional UB-04 Claims through a clearinghouse, you can set up the claim transmission details.

  • Go to BillingTracker > Setup > Electronic Claim Transmission Setup.
The default settings work with ClinicTracker Partners. For other clearinghouses, please consult your Clearinghouse Companion Guide for appropriate settings, or reach out to your clearinghouse directly.

Adding the UB-04 Option

If you send claims using the UB-04 form, you'll want to enable the UB-04 Option.

  • Go to ClinicTracker > Admin Config >Options > Admin tab.
  • Check the box for Display UB-04 Support Fields.
  • Click OK.

Setting Your Up UB-04 Form Configurations

If your clinic sends claims using the UB-04 form, you'll want it to meet the specifications of your payors.

  • Go to BillingTracker > Setup > UB-04 Configuration
  • Adjust the fields according to your specifications.
  • Click OK.

Applying UB04 Form Settings to Insurers

After creating your UB-04 configurations, you will want to apply certain configurations to specific insurers and set insurer-specific options.

  • Go to BillingTracker > Setup > Insurer Plans > Claim Detail tab.
  • Adjust the fields according to your specifications.
  • Click OK.

Setting Up Your Revenue Codes 

This form allows you to configure how to display revenue code/CPT code information for your payors/Third Party Administrators on the UB-04 form.

  • Go To BillingTracker > Settings > Revenue Code Setup.
  • Adjust the fields according to your specifications.
  • Click OK.

Adjusting Your UB04 Print Settings

If your UB-04 does not print properly, adjust the print settings.

  • Go to BillingTracker > Setup > Options.
  • Adjust as needed.
  • Click OK.
Tip: Pixels are a very small unit of measurement. When adjusting your settings try incrementing the margins in 50 unit increments, then use smaller increments to fine-tune.
