Demographics Form Settings
- Require Referral Source: This makes 'Referral Source' a Required Field (Intake Tab of Demographics Screen)
- Restrict Entries: Referral Source: Limit selection to only list values (don't allow free-text)
- Require Referred By: This makes 'Referred By' a Required Field (Intake Tab of Demographics Screen)
- Require Referral Phone Number: This makes 'Referral Phone Number' a Required Field (Intake Tab of Demographics Screen)
- Require Referral Email: This makes 'Referral Email' a Required Field (Intake Tab of Demographics Screen)
- Require Referral Reason: This Makes Reason for Referral a Required Field (Intake Tab of Demographics Screen
- Restrict Entries: Referral Reason: Limit selection to only list values (don't allow free-text)
- Require DOB: This Makes DOB a Required Field on the Demographics Tab
- Hide Packet Dates: Prevents the Packet Date field from showing on the Intake Tab
- Require Client ID: This Makes Client ID (which may be renamed as Hospital Number, Medical Record #, etc.) a Required Field on the Intake Screen
- Client ID - Prevent free-text entry: Enable this setting to prevent a user from manually typing in a Client ID (which may be renamed as Hospital Number, Medical Record #, etc.), which will require having the system calculate all new values.
- Automatically Generate Client ID: Enable this setting to have the Client ID (which may be renamed as Hospital Number, Medical Record #, etc.) automatically generated on the first save of a new patient.
- Automatically Open Insurance Eligibility Verification: Enable this setting to have the Insurance Eligibility Verification form open automatically for each insurance selected (primary, secondary, and tertiary) on the first save of a new patient.
- Automatically Review History for Insurance Eligibility Verification: Enable this setting to have the Insurance Eligibility Verification form open to the Review History tab and automatically refresh the data when verifying eligibility (Insurance tab of Demographic screen). This option will not apply when automatically opening the form when a new patient is saved for the first time because there will likely be no history.
- Date Entered - Prevent free-text entry: Enable this setting to prevent a user from manually adjusting the value, which will ensure the default current date will always be used..
- Display Client Characteristics: Display a dropdown for Client Characteristics on the Intake tab of the Intake/Patient Info form
- Require Social Security Number: This Makes Social Security Number a Required Field on the Demographics Screen
- Require Intake By: This Makes Intake By a Required Field on the Intake Screen
- Require Marital Status: This Makes Marital Status a Required Field on the Demographics Screen
- Require Contact Method: This Makes Preferred Contact Method a Required Field from Check box on the Demographics Screen
- Restrict Entries: Counties: Limit selection to only list values (don't allow free-text)
- Require Race: This Makes Race a Required Field on the Demographics Screen
- Require Insurance Policy Number: Enabling this setting will require a Policy Number to be entered if a payor is selected on the primary, secondary, or tertiary Insurance tabs.
- Require Insurance Policy Holder and Employer when Payor is Selected: Enabling this setting will require these fields to be entered if a payor is selected on the primary, secondary, or tertiary Insurance tabs.
- Require Group Number when Payor is Selected: Enabling this setting will require the Group Number to be entered if a payor is selected on the primary, secondary, or tertiary Insurance tabs.
- Require Contact Notes: This Makes Contact Notes a Required Field on the Demographics Screen
- Display Parent Tab for Adults: Display a tab to collect parent information even if client is marked as an adult
- Disable Duplicate Checking: Eliminates prompts for patients with duplicate name, DOB, or SSN.
- Require Family Info for Children: This makes both family composition and marital status required for clients marked as "child"
- Default Duration of Consents Months: Default duration of authorizations recorded on Info Releases tab.
- Age Cut-Off for Adults: This value determines the age at which a patient is considered an adult (default is 18)
- Require Primary Place of Service: This Makes Primary Place of Service a Required Field on the Intake Screen
- Require County of Residence: This Makes County of Residence From a Required Field on the Intake Screen
- Require County of $ Responsibility: This Makes County of Financial Responsibility a Required Field on the Intake Screen
- Require Intake Clinician: This Makes Intake Clinician a Required Field from Drop Down Selection on the Intake Tab
- Require Ethnicity: This Makes Ethnicity a Required Field on the Demographics Screen
- Require Additional ID: This Makes Additional ID a Required Field on the Intake Screen
- Require Follow-up Clinician: This Makes Follow-up Clinician a Required Field on the Intake Tab
- Require Heard About Us From: This Makes Heard About Us From a Required Field on the Intake Tab
- Require Primary Language: This Makes Primary Language a Required Field on the Demographics Screen
- Require Sexual Orientation: This Makes Sexual Orientation a Required Field on the Demographics Tab
- Require Date Packet Sent: This Makes DatePacketSent a Required Field on the Intake Tab

- Require Date of Contact for Appointment: This Makes Date of Contact for Appointment a Required Field on the Intake Tab

- Require Client Characteristics: Enabling this setting makes the field required on the Intake tab
- Require Intake Service: Enabling this setting makes the field required on the Intake tab
- Display Additional Patient Groups: Enabling this setting displays the Additional Patient Groups list on the Intake tab
- Require Email Address: Enabling this setting makes the field required on the Demographic tab
- Hide Discharge Status: Prevent the Discharge Status field from showing (even for discharged patients).
- Require Advanced Directive: This Makes Advanced Directive a Required Field on the Demographics Screen
- Require Gender Identity: This Makes Gender Identity a Required Field on the Demographics Tab
- Require Intake Disposition: This Makes Intake Disposition a Required Field on the Intake Tab
- Require Parental Marital Status: This Makes Parental Marital Status a Required Field on the Parent/Guardian Tab
- Require Number of Children in Family: This Makes Number of Children in Family a Required Field on the Parent/Guardian Tab
- Require Number of Children in Home: This Makes Number of Children in Home a Required Field on the Parent/Guardian Tab
- Require Number of Children in Same School: This Makes Number of Children in Same School a Required Field on the Parent/Guardian Tab
- Require Emergency Contact Info: This Makes Emergency Contact Info a Required Field on the Outside Providers/Contacts Tab
- Require Preferred Contact Method: This Makes Preferred Contact Method a Required Field on the Demographics Tab
- Require parent's information on the Parent/Guardian tab if that parent is the insured party: When saving an insurer, if the relationship to insured references the mother or father, the system will display a prompt if any of that parent's information is missing.
- Require Cell Phone Number: This Makes Cell Phone Number a Required Field on the Demographics Tab
- Disable Patient Portal Account on Discharge: When discharging a patient, this setting determines if the patient's portal account (as well as any authorized representative access) always gets deactivated, never gets deactivated, or if a prompt is displayed to decide at that time.
- Hide Emergency Contact Info Panel
- Remove Prompt when Disassociating Patient Portal Usernames: When clicking the Disassociate button on the Patient Portal Account Setup screen, check this option to bypass the prompt to the user.
- Remove Intake Option: If enabled, all clients will be entered as 'Patient' without the option for 'Intake' status in the Type dropdown.
- Hide Homeless Checkbox: Enable this setting to prevent the Homeless checkbox from displaying on the Demographic form.
