- Adjust Form Type For: This gives you the option of choosing between Outpatient Services and In-Home Services. Depending on your choice, certain forms will be adjusted accordingly (e.g. If In-Home Services is selected, the Intake form will show a Priority Code field on the Demographics tab and the Compliance module will display different requirements).
