Forms

This page displays custom forms available to a staff member. As such, it functions similarly to the report at:  Paperwork -> MH Paperwork/CD paperwork in ClinicTracker:

1. These columns indicate whether the form is for Mental Health (MH), Chemical Dependency (CD), or both. Clicking the blue MH/CD link next to an item will open that form.
2. Here you'll find the form's name.
3. A brief description of the form's purpose (if it was entered).

Note: Supervisors can use the Staff Portal to sign forms, however, this is limited to signing Custom Forms. To sign Stock Forms the supervisor would need to access ClinicTracker and sign directly from within the application.
