Existing Clinics:

To add a new payor or a new provider, existing Billing Service customers may use these online forms.

New Clinics:

If your clinic is new to our Billing Service, you will be asked to provide us with important details using the forms below. This information will allow the Billing Service team will to configure your Clinic correctly in ClinicTracker, create the procedures for your clinic's billing, submit payors for EDI and ERA enrollment through your clearinghouse, and link providers to your practice in your clearinghouse.

Note: Once completed and saved to your local computer, this file can be sent as an attachment to a Support Request. You may open a support request at help.clinictracker.com using the blue support tab on the right-hand side.
