Online Forms

• Allow Users to Complete and Return Online Forms: Check this box to allow Portal Users to be able to access and complete Custom Patient Forms requested the clinic.
• Set Default Due Date for Online Forms: Enter the number of days before Patient Portal Form Requests become due. Same day (0), Next Day (1), One Week (7).
• Set Record Date to Submission Date for Patient Portal Forms: Check this box to set the record date to the current date each time a Portal User submits a form.
• Enable Open Forms in Patient Portal: Enable Open Forms in Patient Portal
• Allow Editing of Open Forms: Check this box to allow Portal Users to edit their saved 'Open Form' records in Patient Portal.
• Show Form Requests for Inactive Treatment Phases: Enable this setting to display form requests, even if they were made within a Treatment Phase that has since been discharged.
• Allow viewing of submitted Patient Portal forms: Enabled forms submitted through the Patient Portal to be available in Read Only mode.
